We are only able to dispense repeat prescriptions for patients already receiving treatment from the vets at Animed.
We need to identify your clinical records on our computer system and confirm with the veterinary surgeon in charge of your case that continued medication is suitable.
Please submit additional forms if you require more than one medication.
Dispensing of prescription medicines is strictly controlled and we can only supply exactly what is entered on the following form. If you have any queries or concerns please contact us by phone or in person. All entries marked with * must be completed.
You will find the information you need on the Animed label attached to the current medication.
We require at least 48 hours notice (two working days) to make sure prescription medicines are correctly supplied and we will email you when your request is ready unless you ask us to text you.
